Who submits the claim or Pretreatment Estimate?
If your plan has the PPO option and you go to a participating dentist or specialist they will submit claims and Pretreatment Estimates for you. However, if you see a non-participating dentist or specialist you will need to contact your dental office to see if they will submit the claims or Pretreatment Estimates for you.
